I. CONCEPT OF ALLERGY

Allergy is an abnormal state of a body when exposed to an allergen (antigen) for the second time and subsequent times. Allergy can cause mild reactions, quickly disappearing so it is easy to ignore or can occur violently leading to anaphylactic shock, causing death such as penicillin allergy. Allergens can be food, plants, cosmetics, disease prevention and treatment drugs. Currently, the number and types of drugs are increasing, the quality of some drugs is not guaranteed, moreover, people with chronic diseases use the same drug many times or have many drugs with similar formulas, so the complications of drug allergies are increasing.

Thus, allergies must have antigens introduced from the outside and antibodies produced by the body. When the antibodies encounter the antigen for the second time, histamine is released from the cells. Allergy symptoms are clinical manifestations of histamine such as itching, hives, rash, eczema, asthma...

II. CLASSIFICATION : usually divided into 2 types

- Natural antihistamines such as histaminease enzymes (extracted from kidneys, liver, lungs) for example: Adrenaline, ephedrine, theophylline.

- Antihistamine H, synthetic such as chlorpheniramine, promethazine, dimedrol.

III. MECHANISM OF ACTION

Antihistamine H, synthetic, works by competing with histamine at the H receptor (located in blood vessel walls, smooth muscle of the bronchi, intestines, uterus...). The drug removes histamine from the ovary, thereby inhibiting the manifestations of histamine.

H histamine drugs only treat the symptoms, not the cause, so it is necessary to find the cause of the disease and combine it with other drugs for effective treatment.

IV. PRINCIPLES OF USE

- Use the medicine early, prolong the maintenance dose. When taking the medicine, just swallow, do not chew.

- Need to combine with drugs to treat the cause of the disease. Do not use for people who are operating machinery or vehicles because the drug has the effect of causing drowsiness.

V. COMMONLY USED DRUGS

1. Promethazine: (Phenergan, pipolphen)

- Properties: White crystalline granules, odorless, bitter taste that numbs the tongue, easily soluble in water, alcohol, chloroform, insoluble in ether, turns pink when exposed to light.

- Effect: Antihistamine H, strong and long lasting, pain reliever, sleep inducing, antiemetic.

- Side effects: Causes dizziness, dry mouth, causes orthostatic hypotension when injected, causes drowsiness.

- Indications: Allergies of all causes such as rash, hives, edema, allergic asthma, neuralgia, mental disorders, insomnia (should be combined with sedatives, sleep-inducing drugs) and combined with pre-anesthetic drugs in surgery.

- Contraindications: People operating machinery, vehicles, subcutaneous injection (causes irritation).

- How to use, dosage: Adults:

● Oral: 25mg/time, 3 times a day - tablets or sugar-coated tablets 15 - 25mg.

● Intramuscular injection: 25-50mg/time, 1-2 injections per day - injectable form pipolphen 50mg/2ml.

● Slow intravenous injection: 50mg/time combined with injectable pre-anesthetic drugs as above.

Children:

● Take 2.5 - 5mg/time, 2 times a day - 15mg tablets or 1ml promethazine syrup containing 1mg, 2 - 4 times a day, each time according to age.

● Under 24 months old take 1/2 teaspoon.

● Children 2 - 6 years old take 1 teaspoon.

● From 7 to 15 years old, take 2 teaspoons

2. Diphenhydramine (Dimedrol, allergen)

- Properties: White crystalline powder, odorless, bitter taste, hygroscopic in humid conditions, soluble in water, alcohol, insoluble in ether.

- Effect: Antihistamine H, milder than phenergan, sedative, antispasmodic, antiemetic.

- Side effects: Causes dizziness, dry mouth, nausea but will go away when stopping the drug.

- Indications: Allergies of all causes, motion sickness, seasickness, vomiting due to pregnancy, Parkinson's syndrome.

- Contraindications: People operating machinery, vehicles, subcutaneous injection (causes irritation).

- How to use, dosage: Adults:

Oral: 25 - 50 mg/time, 3 times a day - tablet or sugar-coated tablet 10mg.

Intramuscular injection: 10 - 20 mg/time, 2 injections per day - dimedriot injection 10 mg/ml or intravenous drip infusion 10 - 20 mg/time mixed with 0.9% sodium chloride infusion.

Children :

Take 10 - 20mg/time, 3 times a day - in pill form as above.

3. Chlopheniramine (Allergin, trimeton)

- Effect: Antihistamine stronger than promethazine and causes sleep.

- Indications: Allergies of all causes, runny nose, stuffy nose due to spasm and Quincke's edema, allergic conjunctivitis.

- Contraindications: People operating machinery or vehicles.

- How to use, dosage: Adults:

Oral: 4mg/time, 3-4 times a day - Tablet or sugar-coated tablet 4 mg.

Intramuscular injection: 10 - 20mg/time, 1 - 2 injections per day - chlorpheniramine injection 10mg/1ml.

Children:

Take 2mg tablets, 1 - 2 times daily, each time according to age:
