Obstetrics and Gynecology Traditional Medicine Part 2 - 8

1.3. Blood heat

The blood is already deficient, and when giving birth, blood is lost, the yin deficiency causes heat in the blood, or the seven emotions are harmful inside, causing liver depression and heat, forcing the blood out.

2. SOLUTIONS

The clinical symptoms of this disease are mainly manifested in the blood sweat. When treating, it is necessary to distinguish between blood deficiency or excess, cold or heat. In addition to the general symptoms, attention should also be paid to the color, properties, quantity, and odor of the blood sweat. If the blood color is pale red and abundant, it is mostly due to qi deficiency; if the blood color is purple-black and accompanied by lumps, it is mostly due to blood heat. It is also necessary to examine the pulse and symptoms carefully, then it is not difficult to distinguish between cold, heat, blood deficiency, and excess. Common symptoms are as follows:

2.1 Vaginal discharge

After giving birth, the goat's blood does not go away, the complexion is pale with sticky substance, no foul smell, the waist and back are tired, the abdomen is distended, the heart is upset, the qi is short, sometimes the lower abdomen is seen to fall, the spirit is tired, the appetite is poor, the tongue is pale, the tongue coating is normal, the pulse is slow and weak; if the blood is deficient, the complexion is pale or yellow, the head is dizzy, the eyes are blurred, the ears are ringing, the body and limbs are cold, the tongue is pale without coating, the pulse is weak and thin.

2.2. Blood stasis

After giving birth, there is a lot of foul-smelling blood that does not stop, a lot of blood that is purple-black or accompanied by lumps, lower abdomen pain that does not allow rubbing, lumps appear when using crutches, chest and abdomen feel distended and painful, face is purple, no desire to eat, bowel movements are sometimes congested, severe constipation, fever, delirium, loose urination, slightly purple tongue, weak, rapid or rapid pulse, strong.

2.3. Blood heat syndrome

After giving birth, the blood is red and has a fishy smell. Sometimes the abdomen is distended, the face is red, the mouth and tongue are dry, the tongue is pale, the pulse is weak and strong, as if the liver is hot and depressed, the flanks are distended and painful, the spirit and body are depressed, the mouth is dry, the heart is upset, the tongue coating is yellow, the pulse is deep and strong.

3. TREATMENT

The treatment for persistent blood odor still follows the principle of "blood deficiency needs to be toned, blood stasis needs to be strengthened, and blood heat needs to be cleared". If blood deficiency cannot be retained, then it is necessary to replenish Qi and regulate blood, using the formula Bo trung ich khi thang.

(1); because of blood stasis, it is necessary to clear the blood and remove the stasis, using the Sinh Hoa thang formula.

(2); if yin deficiency and blood heat occur, one should nourish yin and clear heat, using the Bao am tien formula (3). In addition, this disease often has excess in deficiency, and excess in blood deficiency. Clinically, one should make a careful diagnosis, either using blood tonifying methods while also using blood circulation methods, or using blood circulation methods while also using blood tonifying methods. Apply flexibly to avoid mismatching with the disease's mechanism.

4. SUPPLEMENTARY METHOD

POSTPARTUM FEVER

; Vi' J • ) ị ị , - »

Fever is a symptom that can appear in many diseases. This article is about fever after giving birth, so it is called postpartum fever. In the book "Jin Gui Yao Luoc" (The Nature of Postpartum Fever), it is written: "Postpartum stroke, fever, red face, asthma and headache, using Zhu Ye Tang as the main treatment." This is about postpartum fever.

1. CAUSES OF DISEASE

The causes of postpartum fever can be summarized into 5 types: external factors, blood deficiency, yin deficiency, blood stasis and food stagnation.

1.1. Extrasensory perception

After giving birth, the blood and qi are damaged, and external evils take advantage of the damaged qi to invade.

enter.

1.2. Blood deficiency

Too much blood loss, yin blood is depleted, yang has no place to rely on, scattered outside.

1.3. Void

The yin part is already depleted, and with postpartum blood loss, the yin blood is deficient, causing evil fire to flourish in the internal organs, leading to the blood being burned by that heat.

1.4. Blood stasis

After giving birth, the blood does not flow down, blood stasis is blocked inside, causing the nutrition and defense systems to be out of balance.

1.5. Reality

• •

After giving birth, working too soon and being tired damages the spleen, making it unable to metabolize nutrients, or eating too many fatty and sweet foods, which accumulate in the stomach.

2. SOLUTIONS

2.1. Extrasensory perception

After giving birth, there will be a feeling of extreme heat and chills, headache, body aches, back pain, dry mouth, no thirst, no sweating, pale tongue with white coating, floating pulse. If evil invades the Yangming meridian, then the high temperature will not be relieved, the face will be red and swollen, asthma with headache, floating and rapid pulse.

2.2. Blood deficiency

After giving birth, I had a slight fever, my face was flushed, I was sweating profusely, my head and eyes were dizzy, my ears were ringing, my heart was pounding, my tongue was pale, my tongue had a thin white coating, and my pulse was large and weak.

2.3. Yin deficiency

After giving birth, fever and headache, not afraid of wind, chills, hotter in the afternoon, red cheeks, thirsty mouth, like to eat and drink cold food, heart palpitations, night sweats while sleeping, constipation, yellow or red urine, thin and rapid pulse.

2.4. Blood stasis

After giving birth, there is fever, the blood does not come down or comes down very little, the blood color is gray-purple, accompanied by blood clots, lower abdomen is distended and painful, cannot be pressed, constipation, thirsty mouth, does not want to drink water, tongue is normal or slightly purple, pulse is deep and waxy.

2.5. Evidence of stagnation

Postpartum fever, chest tightness, belching, sour swallowing, loss of appetite, or upper abdominal distension and pain, or vomiting or diarrhea with food, normal tongue, thick, viscous coating, and active pulse.

3. TREATMENT

Although fever after childbirth is mostly due to deficiency symptoms and rarely due to excess symptoms, the treatment cannot be based on just replenishing deficiency symptoms, but must also consider the cause to discuss treatment. In the case of postpartum treatment, if there is evidence of excess symptoms, the first thing to do is to use Thua Khi thang, and if there is evidence of excess symptoms, then use that medicine, and cannot be bound by taboos and delay the disease. However, regarding taboos, one must also pay attention.

As for the specific treatment, dialectical treatment is still the main method. If there is an external symptom, one should nourish blood and eliminate wind using Jiawei Siwu Tang (1) or Zhuye Tang (2); if there is a large loss of yin and yang energy, one should replenish blood and consolidate qi using Danggui Buxue Tang (3); if there is yin deficiency and heat in the blood, one should replenish yin using Digu Pi Yin (4); if there is blood stasis, one should clear blood and eliminate stagnation using Shenghua Tang (5) with added Danshen and Honghua; if there is food stagnation, one should nourish the spleen and clear stagnation using Liujunzi Tang (6) with added Shanzha and Shenqu; if there is no deficiency but there is food accumulation, one should eliminate food accumulation and use Jiawei Pingwei San (7) to treat.

POST-PRODUCTION GLASSES

After giving birth, the back suddenly stiffens, the limbs convulse, and in severe cases, the person can't speak, which is called postpartum tetanus. Although the symptoms are similar to internal medicine's tetanus, the cause and treatment are different, so they must be placed in a separate subject. As for symptoms that fall under the scope of internal medicine, they will not be mentioned here.

1. CAUSES OF DISEASE

The common cause of postpartum hemorrhage is excessive blood loss and loss of body fluids. Based on the common symptoms seen in the clinic, it can be divided into two types: blood deficiency and stroke.

1.1. Blood deficiency

Postpartum blood loss and fluid loss cannot moisten the meridians, causing the Du meridian to lose nourishment and the bones to stiffen.

1.2. Stroke

Postpartum, the blood and qi are depleted, the pores are not open, and the wind-evil invades and damages the Taiyang meridian, causing the meridians to contract. Or, the external evil enters the interior and turns into dryness, giving rise to wind.

2. SOLUTIONS

Postpartum glaucoma is basically caused by dry blood and different external wind evils, so the symptoms can be differentiated, and at the same time there are also multiple symptoms, so it is necessary to differentiate clearly.

2.1. Blood deficiency

Postpartum bleeding causes sudden onset of chills, stiff neck, clenched teeth, pale or yellow complexion, convulsive limbs, pale red tongue without coating, weak pulse; if blood and qi are lost , the body becomes stiff , limbs are cold, hands and feet are slightly spread out, shortness of breath and sweating, glaring eyes, uncontrolled bowel movements, and a floating, large, lifeless pulse.

2.2. Stroke

Postpartum with wind-evil cold, stiff neck and head, chills and fever, body aches and backache, then stiff limbs. Teeth clenched, pale tongue, thin white coating, floating and wiry pulse: if the symptoms are reduced but the internal organs are very hot, then the body is hot, mouth thirsty, face flushed, lightheaded, both hands clenched, constipation. Short red urine. Red tongue, thin yellow coating, deep and wiry pulse.

3. TREATMENT

In order to treat this disease, it is necessary to pay attention to the characteristic of excessive postpartum blood loss and yin deficiency . Therefore, even if the phenomenon is wind, one should still follow the ancient theory of "treating wind, one must first treat blood, when blood circulates, wind will naturally disappear" to proceed with treatment; if one simply uses medicine to expel wind, it will lead to consequences.

not good. Specifically about

Treatment: If blood is deficient , then one should replenish blood and qi , and also expel wind using the formula Si vinh hoat lac thang (1). If the deficiency is too severe and about to develop into a hernia, then one should use Doc sam thang (2). If wind invades from the outside, then one should clear the blood and eliminate wind, and additionally dispel the exterior evil using the formula Hua da du phong tan (3). If the exterior evil has not yet been eliminated, the internal heat is strong, and the strong heat will certainly generate wind, then one should dispel the exterior and clear it, and add a medicine to dispel wind using the formula Linh duong giac am tu (4).

POSTPARTUM DIFFICULTY

After giving birth, eating and drinking as usual but not having a bowel movement or constipation or not having a bowel movement for several days, is called the condition of having difficulty passing stool. The book "Jin Gui Essentials for Women's Health" says: "Women who have just given birth have three diseases, the third of which is having difficulty passing stool." After recovering from the illness, they can eat, but after 7 or 8 days they develop a fever, which is due to heat in the stomach, so use the Great Cheng Qi Decoction. "After 7 or 8 days of giving birth, there is no Tai Yang syndrome... but no bowel movement, feeling restless and hot, the pulse is slightly heavy, and the fever increases even more, in the afternoon they feel restless and do not eat, when they eat they talk nonsense, but at night they get better, so use the Great Cheng Qi Decoction..." These are all methods to treat difficulty passing stool after giving birth.

1. CAUSES OF DISEASE

Mainly because after giving birth, there is a lot of blood damage, the stomach and intestines are weak, and there is not enough body fluid, causing the disease. But occasionally there is evidence of yangming stomach excess, the analysis is presented as follows:

1.1. Blood deficiency

During childbirth, too much blood is lost, blood deficiency causes body fluids to be depleted, and cannot moisten the intestines, or blood deficiency causes heat to burn the body fluids inside.

1.2. Real taste

The external evil of the tapeworm enters the interior, or food and drink accumulate, heat accumulates inside, the intestines are dry, and the taste is full.

2. SOLUTIONS

2.1. Blood deficiency

Postpartum women have difficulty defecating, or many days without bowel movements, yellow complexion, dry skin, not swollen abdomen, normal eating and drinking, pale tongue, weak and thin pulse, if there is internal heat, dry mouth, full chest, swollen abdomen, red tongue, thin yellow coating, thin and rapid pulse; if there is qi deficiency, shortness of breath, spontaneous sweating, dizziness, fatigue, large and weak pulse.
